Travel Nurse Tax & Stipend Basics

Travel-nurse pay packages can look strange.

You may see:

  • Taxable hourly pay
  • Housing stipend
  • Meals and incidental allowance
  • Travel reimbursement
  • Bonuses

And then someone says:

“The stipends are tax-free.”

That statement is incomplete.

Tax treatment depends on whether you meet the rules for business travel away from your tax home, how the reimbursement is structured, and whether the assignment is temporary.

Important: This is general educational information, not individualized tax advice. For tax-home questions, long extensions or multiple assignments in the same area, use a qualified tax professional familiar with travel healthcare.

The Short Version

For federal tax purposes, the IRS generally treats your tax home as your regular place of business or post of duty—not automatically the house where your mail goes.

If you do not have a regular or main workplace, the IRS uses additional factors.

The IRS also says a temporary assignment in one location is generally one realistically expected to last one year or less. An assignment realistically expected to last more than one year is generally indefinite. citeturn302482search0

What Is a Tax Home?

The IRS says your tax home is generally your regular place of business or post of duty, including the general city or area where you work. citeturn302482search0

If you do not have a regular main workplace, the IRS considers factors including whether:

  1. You perform part of your business in the area of your main home and lodge there while doing business.
  2. You have living expenses at that home that you duplicate while working away.
  3. You have not abandoned the area and maintain meaningful ties there. citeturn302482search0

Why Travel Nurses Care

Tax-advantaged travel reimbursements are based on the idea that you are temporarily working away from your tax home and incurring travel/living expenses.

If you are not actually traveling away from a qualifying tax home, the treatment can change.

Temporary vs. Indefinite Assignments

The IRS generally says:

  • Expected to last one year or less = temporary
  • Expected to last more than one year = indefinite

If circumstances change, the status can change when the expectation changes. citeturn302482search0

The IRS example shows a job initially expected to last nine months becoming indefinite after circumstances change and the worker expects to stay beyond a year. citeturn302482search0

What Happens if the Assignment Becomes Indefinite?

Publication 463 says that if an assignment becomes indefinite, the assignment area generally becomes the worker’s tax home, and employer-paid living allowances can become taxable income even if labeled travel allowances. citeturn302482search0

What Is a Housing Stipend?

A housing stipend is money intended to reimburse or cover lodging costs while traveling for work.

Whether the reimbursement is excludable from taxable wages depends on the underlying rules.

What Is an Accountable Plan?

The IRS uses accountable plan for employer reimbursement arrangements meeting specific requirements.

Generally, the reimbursement must relate to legitimate business expenses, and the employee must adequately account for the time, place and business purpose. Excess reimbursements must be handled according to the plan rules. citeturn302482search0

Per Diem Does Not Mean “Free Money”

Federal per diem rates vary by locality.

Publication 463 points to GSA rates for continental U.S. locations and separate federal sources for Hawaii, Alaska and other non-CONUS locations. citeturn302482search0

An agency stipend is not automatically tax-free merely because it is below a published per diem rate.

Current Federal Per Diem Framework

For FY2026, Publication 463 notes that the standard CONUS rate for locations without a special locality rate remained $178 total:

  • $110 lodging
  • $68 meals and incidentals

Many cities have higher locality-specific rates. citeturn302482search0

The separate high-low substantiation method for the 2025–2026 period uses:

  • $319 for designated high-cost localities
  • $225 for other localities

under the applicable rules. citeturn302482search1

These are federal substantiation figures, not guaranteed tax-free travel-nurse stipend amounts.

Taxable Hourly Rate Matters

Do not ignore the taxable wage.

It can affect:

  • Overtime
  • Social Security wages
  • Medicare wages
  • Unemployment calculations
  • Disability benefits
  • Loan applications
  • Retirement contributions

Get the full package in writing.

Duplicated Expenses

The IRS specifically considers duplicated living expenses as one factor when determining tax-home status for people without a regular main workplace. citeturn302482search0

Keep records.

What Records Should Travel Nurses Keep?

We would keep:

  • Lease or mortgage records
  • Utility bills
  • Assignment contracts
  • Temporary housing leases
  • Rent receipts
  • Travel records
  • Pay packages
  • W-2s
  • Reimbursement records

Publication 463 says travelers should maintain records supporting expenses and employer advances/reimbursements. citeturn302482search0

Multiple Assignments in the Same Area

Be especially cautious if you:

  • Extend repeatedly
  • Move between hospitals in the same metro
  • Spend most of the year in one city

The issue becomes whether that area has effectively become your main place of business.

A brief trip home does not automatically reset the clock.

Stipend vs. Agency Housing

If the agency provides housing directly instead of paying a stipend, compare:

  • Housing value
  • Taxable wage
  • Stipend you would otherwise receive
  • Deposit risk
  • Flexibility
  • Location

Agency housing is not magically free.

Can Travel Nurses Deduct Their Own Unreimbursed Expenses?

Publication 463 notes that most ordinary employees currently cannot use Form 2106 to deduct unreimbursed employee business expenses, except for limited categories such as certain reservists, qualified performing artists, fee-basis officials and employees with impairment-related work expenses. citeturn302482search0

Do not assume you can simply write off whatever the agency does not reimburse.

State Taxes

Federal tax-home rules are only part of the picture.

You may also deal with:

  • Home-state return
  • Work-state return
  • Reciprocity
  • Local taxes

State rules vary.

Hawaii Is Different for Per Diem Tables

The normal GSA CONUS table does not cover Hawaii.

Publication 463 points to Department of Defense per diem sources for Hawaii, Alaska and other non-foreign areas outside the continental United States. citeturn302482search0

Our Verdict

Remember four things:

  1. Your tax home is not automatically your mailing address.
  2. Temporary generally means realistically expected to last one year or less.
  3. A stipend is not automatically tax-free because a recruiter calls it a stipend.
  4. Keep records.

If travel income is a major part of your finances, paying a tax professional who understands travel healthcare is much better than building your tax strategy from social-media comments.
